Better help for your patients

With Life Track Planners, I want to help as many people as possible who are suffering with long-term, chronic, multiple or mystery illnesses.  Because I know the struggles they face from personal experience.  But in order to help the patients, I also feel that it is vitally important to assist the health care professionals taking care of them.

 

Treating someone who has on-going health issues can take up a lot of time, resources and let's be honest, energy.  You only have so much time in an appointment to assess both physical and mental health problems whilst trying to give patients the answers they desperately want and if that patient forgets something, thinks something isn't relevant, or has trouble explaining the issue, it can make it even harder to decide how to help them.  This can lead to multiple appointments, random testing or referrals to other specialists for help.

That's where I believe Life Track Planners can help.  Whilst they are not a diagnostic solution (I leave that down to the amazing health care workers) they are a tool to help patients track their health, so they can bring a better insight into their condition when they go to see their GP or other health professional.  They are designed to cover all areas of their health and even day to day life, in order to get a better overview of what is happening

within their body and also looking at any external factors that may be affecting it.

They can also help during treatment to monitor and track the progress of

the patient and their symptoms.​

In order to help as many people as possible, I want to make my

Life Track Planners as accessible as I can to GP surgeries, hospitals and

any other health care workers who have patients struggling, just like I have,

with their on-going health problems.
