This planner has been created to help people with long term or recurring health issues. It can be so overwhelming to keep track of all the different aspects of our health, both mental and physical, so we've put it all in one place with space for the everyday activities too.
Having details of your physical and mental wellbeing in one place can really help you to see how you are doing day to day and show you areas where you can make positive changes to benefit not only your body but your mind as well.
Studies have shown that your mental wellbeing can have a direct impact on your physical health, a fact that I have experienced within my own life. So now I want to use this knowledge to help others in their journey back to better health.
This digital download version allows you to print off as many weeks as you like. Each week there are sections to help you focus on what is important to you and a weekly summary to help you make sense of how your week has been.
Each day has space for you to track your diet, hydration, sleep, exercise, mood, medications and any observations such as symptoms or measurements, for example Blood Pressure/Blood Sugar Levels etc.
